Understanding Plantar Fasciitis
What Is Plantar Fasciitis?
Plantar fasciitis is inflammation and microtearing of the plantar fascia—a thick band of tissue that runs across the bottom of your foot, connecting your heel bone to your toes. This strong, fibrous structure supports the arch of your foot and acts as a shock absorber during walking, running, and standing. When the plantar fascia becomes irritated, inflamed, or develops small tears from repetitive stress or excessive strain, it causes the characteristic heel pain of plantar fasciitis.
The condition typically develops gradually over time as repetitive stress causes cumulative damage to the tissue. Small tears develop in the fascia, triggering inflammation and pain. The body attempts to heal these tears, but continued stress prevents complete healing, leading to chronic inflammation and progressive tissue degeneration if left untreated.
Plantar fasciitis develops when excessive stress is placed on the plantar fascia. Common causes and risk factors include:
- Biomechanical factors: Flat feet, high arches, abnormal walking patterns, or overpronation place extra stress on the plantar fascia
- Age: Most common between ages 40-60 when the plantar fascia loses elasticity
- Activity level: Sudden increases in activity, particularly running or jumping, can overload the tissue
- Occupation: Jobs requiring prolonged standing on hard surfaces significantly increase risk
- Weight: Excess body weight increases stress on the plantar fascia
- Footwear: Shoes with poor arch support or inadequate cushioning contribute to development
- Tight calf muscles and Achilles tendon: Limited ankle flexibility increases strain on the plantar fascia
Common Plantar Fasciitis Symptoms
Plantar Fasciitis Symptoms We Treat
Recognizing the symptoms of plantar fasciitis is the first step toward getting effective treatment. If you're experiencing any of the following, it's time to schedule an evaluation with Dr. Priya:Stabbing Heel Pain
Sharp, stabbing pain on the bottom of the foot near the heel, typically worse with the first steps after waking or after periods of rest (called post-static dyskinesia)
Morning Heel Pain
Severe pain with the first steps out of bed in the morning, which may improve somewhat as you walk around and the tissue warms up, but often returns after
prolonged standing or activity
Pain After Rest
Pain with Activity
Discomfort or pain that worsens with prolonged standing, walking, climbing stairs, or going up on your toes; often worse at the end of a long day on your feet
Heel Tenderness
Tenderness to touch on the bottom of the heel, particularly near the inside edge where the plantar fascia attaches to the heel bone; may have associated swelling or warmth
Limited Flexibility
Decreased range of motion in the foot and ankle, tight calf muscles, difficulty pulling the foot upward toward the shin, and stiffness in the affected foot
Plantar fasciitis symptoms often start gradually and worsen over time if left untreated. The sooner you seek treatment, the faster you can achieve relief and prevent the condition from becoming chronic. Don't wait until the pain becomes unbearable—early intervention leads to better outcomes.
PLANTAR FASCIITIS TREATMENT OPTIONS
Comprehensive Treatment for Lasting Heel Pain Relief
At NJ Sports Spine and Wellness, we take a multifaceted approach to plantar fasciitis treatment. Rather than relying on a single therapy, we combine multiple evidence-based treatments to address inflammation, promote healing, correct biomechanical factors, and prevent recurrence. Dr. Priya will conduct a thorough evaluation including physical examination and imaging to determine the severity of your condition and identify contributing factors, then recommend a personalized treatment plan utilizing the optimal combination of therapies for your situation.
Shockwave Therapy for Plantar Fasciitis
Shockwave therapy, also called extracorporeal shockwave therapy (ESWT), is one of the most effective treatments for chronic plantar fasciitis. This advanced, non-invasive therapy uses acoustic waves to stimulate healing at the cellular level. For plantar fasciitis patients, shockwave therapy breaks up scar tissue and calcifications in the plantar fascia, stimulates neovascularization (new blood vessel growth) to improve circulation, triggers the release of growth factors that promote tissue regeneration, reduces pain through effects on nerve endings, and accelerates the body's natural healing processes. Clinical studies show that shockwave therapy provides significant pain reduction and functional improvement in 70-90% of patients with chronic plantar fasciitis, even those who haven't responded to other conservative treatments. Treatment typically involves 3-6 sessions performed over several weeks. Many patients notice improvement after just a few treatments, with continued progress over the following months as healing occurs. Shockwave therapy is particularly effective for chronic plantar fasciitis that hasn't responded to rest, stretching, or orthotics.
Laser Therapy for Inflammation & Pain
Our therapeutic laser treatment uses specific wavelengths of light to penetrate deep into the plantar fascia and surrounding tissues, promoting healing at the cellular level. Laser therapy for plantar fasciitis reduces inflammation by decreasing inflammatory cytokines, increases ATP production in cells providing energy for healing, improves blood flow to deliver nutrients and oxygen to damaged tissue, reduces pain through effects on nerve conduction, and accelerates tissue repair and regeneration. This painless, non-invasive treatment requires no downtime and can be performed in just minutes. Many patients experience immediate pain relief after laser treatment, with cumulative benefits building over multiple sessions. Laser therapy is especially effective when combined with other treatments like shockwave therapy or physical therapy, creating a synergistic effect that enhances overall outcomes.
Ultrasound Therapy
Therapeutic ultrasound uses high-frequency sound waves to deliver deep heat to the plantar fascia and surrounding tissues. This treatment increases blood circulation to promote healing, reduces muscle spasm and tension in the foot and calf, promotes tissue flexibility and extensibility, accelerates the repair of damaged tissue, and provides pain relief. Ultrasound therapy is particularly helpful for breaking down scar tissue that may have formed in chronic plantar fasciitis and for preparing tissues for stretching exercises. The deep heating effect helps improve the effectiveness of subsequent stretching and manual therapy. Combined with our other treatments, ultrasound therapy enhances overall healing and pain reduction.
Custom Orthotics & Biomechanical Correction
Abnormal foot mechanics are often a primary cause of plantar fasciitis, making custom orthotics an essential component of effective treatment. Dr. Priya will conduct a comprehensive biomechanical assessment of your feet, ankles, and gait pattern, then design custom orthotics specifically for your needs. Custom orthotics provide proper arch support to reduce strain on the plantar fascia, cushion the heel to absorb shock, redistribute pressure across the foot more evenly, correct pronation or supination issues, and support optimal foot alignment. Unlike over-the-counter inserts, custom orthotics are precisely crafted based on a mold or scan of your feet, ensuring perfect fit and optimal correction. They're essential not just for treating current symptoms but for preventing recurrence once your plantar fasciitis heals. Dr. Priya will also provide guidance on appropriate footwear to maximize the benefit of your orthotics and support long-term foot health.
Physical Therapy & Rehabilitation
Injection Therapy
For acute or severe plantar fasciitis pain, Dr. Priya may recommend targeted injection therapy. Treatment options include corticosteroid injections to reduce inflammation and provide significant pain relief, platelet-rich plasma (PRP) injections to stimulate healing using your body's own growth factors, or other regenerative medicine approaches depending on your specific condition. While injections provide substantial pain relief, they're typically used as part of a comprehensive treatment plan that includes physical therapy, orthotics, and other modalities to address the root cause of the problem. Dr. Priya uses precise, ultrasound-guided injection techniques to ensure accurate placement and optimal effectiveness while minimizing discomfort.
Our Multifaceted Approach to Plantar Fasciitis
Research consistently shows that multifaceted treatment approaches produce better outcomes for plantar fasciitis than single-modality treatments. That’s why we integrate multiple evidence-based therapies into your personalized care plan. For example, a typical treatment plan might combine shockwave therapy to break down scar tissue and stimulate healing, laser therapy to reduce inflammation and pain, custom orthotics to correct the biomechanical factors that caused the problem, physical therapy to improve flexibility and strength, and home exercise programs to accelerate healing. This comprehensive approach addresses plantar fasciitis from every angle, leading to faster, more complete relief and better long-term outcomes. Best of all, everything you need is conveniently located under one roof, with all your providers communicating and coordinating your care.
Advanced Treatment for Severe or Chronic Cases
While most patients respond well to conservative treatment, some cases of severe or chronic plantar fasciitis may require more advanced interventions. Dr. Priya offers several options for resistant cases including focused shockwave therapy protocols for stubborn cases, regenerative medicine injections like PRP or amniotic fluid, minimally invasive procedures when appropriate, and surgical options for the rare cases that don't respond to conservative care. However, surgery is rarely necessary with our comprehensive treatment approach. The combination of advanced technologies, expert manual therapy, custom orthotics, and patient education resolves the vast majority of plantar fasciitis cases without surgical intervention.

What to Expect
Your Plantar Fasciitis Treatment Journey
Understanding what happens during plantar fasciitis treatment helps you feel confident about seeking care. Here’s what you can expect when you choose NJ Sports Spine and Wellness:
Initial Consultation & Evaluation
Your first visit includes a comprehensive assessment to identify the cause of your heel pain and develop an effective treatment plan. Dr. Priya will review your medical history, symptom onset, and activity level, conduct a thorough physical examination of your feet, ankles, and lower legs, perform biomechanical assessment including gait analysis, evaluate your current footwear, take in-house X-rays if needed to rule out other conditions like heel spurs or stress fractures, and assess flexibility, strength, and range of motion. This thorough evaluation ensures we understand all the factors contributing to your plantar fasciitis so we can design the most effective treatment approach.
Personalized Treatment Plan
Based on your evaluation, Dr. Priya will explain your diagnosis and the factors contributing to your condition, discuss all appropriate treatment options, recommend a specific combination of therapies tailored to your situation, set realistic expectations for your recovery timeline, answer all your questions, and provide clear guidance on next steps. Your treatment plan may include multiple therapies working together synergistically for optimal results. You’ll understand exactly what each treatment does and how it contributes to your healing.
Treatment Course
- Shockwave therapy sessions: Usually 3-6 treatments performed 1-2 weeks apart
- Laser and ultrasound therapy: Often performed 2-3 times per week initially
- Physical therapy visits: Typically 2-3 times weekly for several weeks, with gradual reduction as you improve
- Custom orthotics: Fitted and adjusted as needed to ensure optimal support
- Home exercise program: Daily stretching and strengthening exercises to accelerate healing

Many patients notice significant improvement within 2-4 weeks, with continued progress over the following 2-3 months. Chronic plantar fasciitis that’s been present for many months may take longer to fully resolve, but consistent treatment following our comprehensive protocol produces excellent results even in stubborn cases.
Recovery & Long-Term Management
As your plantar fasciitis heals, we’ll provide guidance on gradually returning to normal activities, continuing home exercises and stretching to maintain flexibility and strength, proper footwear selection for different activities, using orthotics long-term to prevent recurrence, and recognizing early warning signs if symptoms begin to return. Our goal isn’t just to eliminate your current heel pain—it’s to ensure you stay pain-free long-term by addressing the underlying biomechanical factors that caused plantar fasciitis in the first place.
Expected Outcomes
With our comprehensive, multifaceted approach, most patients experience significant pain reduction, improved foot function and mobility, ability to return to normal activities including exercise and sports, prevention of recurrence through biomechanical correction, and elimination or significant reduction of morning heel pain. While results vary based on severity and duration of your condition, our integrated treatment approach consistently produces excellent outcomes even for patients whose plantar fasciitis hasn’t responded to previous treatments elsewhere.

Frequently Asked Questions
Frequently Asked Questions About Plantar Fasciitis TreatmentRecovery time varies depending on the severity and duration of your plantar fasciitis and how consistently you follow your treatment plan. With our comprehensive approach combining shockwave therapy, laser treatment, physical therapy, and orthotics, many patients experience significant improvement within 2-4 weeks and substantial relief within 2-3 months. Acute plantar fasciitis caught early often responds more quickly, while chronic cases present for many months may take longer to fully resolve. The key is addressing all contributing factors and following through with treatment consistently. Most patients who complete our protocol achieve full resolution of symptoms.
Shockwave therapy, or extracorporeal shockwave therapy (ESWT), uses acoustic waves to stimulate healing in damaged tissue. For plantar fasciitis, shockwave therapy breaks down scar tissue and calcifications in the plantar fascia, triggers the formation of new blood vessels to improve circulation, stimulates the release of growth factors that promote healing, reduces pain by affecting nerve endings, and accelerates your body’s natural repair processes. Clinical research shows shockwave therapy provides significant pain reduction in 70-90% of patients with chronic plantar fasciitis. Treatments are performed in our office and take only minutes. While you may feel some discomfort during treatment, most patients tolerate it well, and there’s no downtime afterward.
It depends on the severity of your condition and the type of exercise. High-impact activities like running and jumping should generally be avoided or significantly reduced while your plantar fasciitis is acute, as they can worsen inflammation and delay healing. However, low-impact activities like swimming, cycling, and upper body workouts can usually be continued. Dr. Priya will provide specific guidance based on your condition. As your plantar fasciitis improves with treatment, you’ll gradually return to your normal activities. Our physical therapists will design a progressive return-to-activity program that allows you to safely resume exercise without risking reinjury. The key is not pushing through severe pain—some discomfort during healing is normal, but sharp pain is a sign to back off.
For most patients, custom orthotics are an essential component of effective plantar fasciitis treatment. Abnormal foot mechanics—such as flat feet, high arches, or excessive pronation—are often primary contributors to plantar fasciitis development. Custom orthotics correct these biomechanical issues by providing proper arch support, cushioning the heel, redistributing pressure, and supporting optimal alignment. Unlike over-the-counter inserts, custom orthotics are precisely designed based on your unique foot structure and gait pattern. They not only help resolve current symptoms but also prevent recurrence by addressing the underlying mechanical problems. During your evaluation, Dr. Priya will assess whether custom orthotics are appropriate for your situation.
While some cases of mild plantar fasciitis may improve on their own with rest and basic self-care, most cases benefit significantly from professional treatment. Without proper treatment, plantar fasciitis often becomes chronic and progressively more difficult to resolve. The longer plantar fasciitis persists, the more difficult it typically becomes to treat. Untreated plantar fasciitis can lead to chronic heel pain, compensatory problems in your gait leading to issues in other joints (knees, hips, back), development of heel spurs, and significantly impaired quality of life. Early professional intervention with our comprehensive treatment approach leads to faster, more complete resolution and prevents the condition from becoming a long-term problem.
While professional treatment is important, there are several things you can do at home to support healing. Ice the affected heel for 15-20 minutes several times daily, especially after activity. Perform plantar fascia stretches before taking your first steps in the morning (Dr. Priya will teach you the proper technique). Stretch your calf muscles and Achilles tendon regularly throughout the day. Wear supportive shoes with good arch support and cushioning—avoid going barefoot or wearing flat, unsupportive footwear. Reduce high-impact activities that aggravate symptoms. Maintain a healthy weight to reduce stress on the plantar fascia. Use night splints if recommended to keep the plantar fascia gently stretched overnight. However, these home measures work best when combined with professional treatment addressing the root biomechanical causes.
Surgery for plantar fasciitis is rarely necessary. The vast majority of patients—over 90%—achieve successful outcomes with conservative treatment. At NJ Sports Spine and Wellness, our comprehensive approach combining shockwave therapy, laser treatment, physical therapy, custom orthotics, and other modalities resolves even stubborn, chronic cases without surgery. Surgery is only considered when conservative treatments have failed after 6-12 months of consistent, appropriate treatment and symptoms remain severe enough to significantly impact quality of life. Even then, minimally invasive procedures are available that have much faster recovery than traditional plantar fascia release surgery. Dr. Priya’s goal is always to achieve resolution with the least invasive approach possible.
While plantar fasciitis can recur, proper treatment that addresses the underlying biomechanical causes significantly reduces this risk. This is why we emphasize custom orthotics, biomechanical correction, and patient education as part of your treatment. By correcting the foot mechanics that contributed to your plantar fasciitis in the first place, we dramatically reduce the likelihood of recurrence. Continuing to wear appropriate footwear with your custom orthotics, maintaining flexibility through regular stretching, managing your weight, and modifying activities that place excessive stress on the plantar fascia all help prevent recurrence. Most patients who follow our complete protocol and recommendations enjoy long-term relief.
Plantar fasciitis and heel spurs are related but not the same condition. Plantar fasciitis is inflammation and microtearing of the plantar fascia. A heel spur is a bony growth that can develop on the heel bone where the plantar fascia attaches. While heel spurs are often present in people with plantar fasciitis, they’re not the cause of the pain—the plantar fascia inflammation is what causes symptoms. In fact, many people have heel spurs visible on X-rays but experience no pain at all. Treatment for plantar fasciitis is the same whether a heel spur is present or not. Our approach focuses on reducing inflammation in the plantar fascia, improving biomechanics, and promoting healing of the damaged tissue.

My experience so far with the rehab team has been incredible. OT Alyssa is the primary therapist I have seen and she is phenomenal at what she does. I appreciate her intentionality with my treatment when we meet 1-on-1. She makes sure to ask how I’m doing and checks in with my progress. A special thanks to Dr. Mayra Tamayo, who has been amazing throughout the entire process. Her support, knowledge, and kindness have made a huge difference and I truly appreciate all the effort she has put into my well-being.
And also thanks to Liam, Tavis, Kat, and Agnies. Your help and support were excellent throughout my recovery. Thank you.

I’ve had an amazing experience at NJ Sports Spine & Wellness! I spent a lot of time researching the perfect place for PT after knee surgery, and I'm glad I came here. The staff is friendly and supportive. Katelynn is a fantastic OT who doesn't rush her patients and has worked wonders on my knee since surgery. Liam is incredibly knowledgeable about ACL surgery recovery and tailors the sessions to my needs. Everyone here pushes me to improve and always makes sure I’m comfortable every step of the way. Everyone is awesome, but I'm glad I get to work with Tori, Alyssa, Agnes, Kat, and Tavis often. I highly recommend NJSS&W for anyone looking for personalized, thoughtful care!
